SCSD Bus P.A.S.S. Template & Procedure
Students who exhibit repeated behavioral challenges on their transportation routes may benefit from having their bus plan documented. In other words, a simplified, driver-friendly version of their ICMP/ ISP-B, including only the actions most essential for promoting student success, would be transferred to a Bus Plan template and shared with SCSD transportation supervisor, Al Valochovic: [email protected] and SCSD transportation assistant, Nicole Stabinski: [email protected] to be uploaded to TransFinder, where bus drivers and bus monitors can access it. Bus Plans must be simple (½ to 1 page long), targeted, specific to things that can be done on the bus, and geared toward bus staff capabilities.

Our ISP-B Template & ISP-B Development Checklist
SCSD's Intervention Support Plan for Behavior (ISP-B) template was updated in September 2016. ISP-Bs are initial problem-solving plans for general education students. They are developed as part of a team-based problem-solving process during School-Based Support Team (SBST) meetings. The ISP-B template aligns very closely to our FBA template (below), and will allow for meaningful team-based problem-solving to occur with the goal of resolving behavioral challenges in the least restrictive environments possible.
We encourage teams to use the ISP-B Development Checklist to stay organized and focused, keeping in mind the specific tasks to complete at each SBST meeting for ISP-B development. We've created an ISP-B Development Checklist with embedded links to all the templates you may need to complete it, including the Daily Behavior Tracking Sheet, ICMP template, Thinking Skills Reference Sheet, customizable Progress Monitoring Spreadsheet, and SCSD Progress Monitoring Report.

Our FBA & BIP Templates & Companion Documents
Functional Behavior Assessments (FBAs) and Behavior Intervention Plans (BIPs) are developed as a team-based process in SCSD, and are completed only as part of a referral to the Committee on Special Education (CSE). For general education students, our district starts with systems-level and classroom interventions, then moves to Intervention Support Plans for Behavior to address individual student needs. If revisions to ISP-Bs prove unsuccessful, teams may wish to consider the potential utility of a formal FBA, as it would shed more light on the function of specific target behaviors, thereby allowing for well-matched intervention strategy identification via BIP. School-Based Support Team (SBST) meetings are used to engage in the problem-solving process and document these efforts via FBA and BIP development. Please view the FBA & BIP Development Checklist (link below) for guidance on specific action items to be completed at each SBST meeting for FBA / BIP development.

Data Collection / Information Gathering Tool Library
These data collection tools & templates provide a great starting point for behavior data collection. They can help teams identify which target behaviors to prioritize in behavior planning efforts and begin to shed light on most common antecedents and maintaining consequences that may contribute to challenging behaviors.
The Daily Behavior Tracking Form is a generic behavioral data collection template that may be a good place to start. It allows for tracking of multiple, pre-coded challenging behaviors and includes an ABC narrative space on page 2. |

Assessment of Lagging Skills & Unsolved Problems (ALSUP) and Thinking Skills Reference Sheet
The ALSUP can help teams identify some lagging skills / unsolved problems and lead to brainstorming about what skills need to be developed for intervention planning purposes. The Thinking Skills Reference Sheet provides a checklist that corresponds to the ALSUP.
These tools are informed by Ross Greene's collaborative problem-solving model and are useful both as part of the Intervention Support Plan for Behavior (iSP-B) development process and as part of the FBA development process. |

Formal Information Gathering Tool Library (for FBA)
The Motivation Assessment Scale (MAS), Functional Analysis Screening Tool (FAST), and Problem Behavior Questionnaire are the three tools we typically choose from that can be used in conjunction with behavioral data & other info to come to a hypothesized function (sensory, tangible, escape, avoidance, or medical) of a problematic behavior. The Questions About Behavioral Function (QABF) is another tool with similar validity and reliability to the MAS. These tools should be completed for each identified target behavior separately (i.e., if there are two identified target behaviors, there should be two MAS’s completed - one for each target behavior - by each person completing the forms).

Progress Monitoring
Here are a few tools for data collection and progress monitoring via creating a visual data summary.
The two word documents (tables) for frequency and duration data collection can be used post-plan development. ISP-B and BIP progress monitoring should track frequency or duration (depending on the target behavior) of target behavior, replacement behavior, and intervention use, and these documents allow for that. The blank, customizable graph template and the progress monitoring spreadsheet can be used to create a visual data summary. |
